Clinic Performance 2021
These are some of my evaluation forms showing my clinical performance, from taking vital signs to instrumentation on dental typodonts and moving up to student patients and on patients who I brought to clinic. These range from my first semester in Junior year to Spring 2022 semester.
Performing these evaluations would prepare me for the next steps in my journey and allow me to focus on the proper technique in order to provide not just adequate clinical care but clinical care that would benefit the health of my patients. It would also prepare me to see patients in the real world and get an opportunity to demonstrate my skills.

Fall 2021 Semester
-
This was one my first ever process evaluations which I had to take vitals on a student.
-
Looking back now and the hundreds of times I have taken vitals, this got me going on a good start after recieving a 4 out of 4, but theres always room to improve.
-
The art of taking vitals is crucial objective evaluation in order to assess how my patient will tolerate treatment or if any issues arise that should be referred to medical doctor for evaluation.
-
After all the safety and health of my patient is always my number one concern
-
This evaluation was for performing instrumentation on a dental typodont in order to get us to build our skills to prepare to see real life patients.
-
Reflecting on these evaluations I am happy to have had the opportunity to get so much feedback in order to perfect the movements, positions, pressures and finger positions in order to be a good clinician.
-
This would ensure the safety and thoroughness of biofilm removal in order to provide the best clinical care possible.
-
This evaluation was my first attempt at exploring for calculus on a patient. I did well except for crossing the line of occlusion.
-
This would be something I would quickly correct and be conscious of whenever I used my explorer to check for calculus. The guidance from professor Schniedewind helped me to improve exploring.
-
The use of an 11/12 explorer is beneficial in order to not only detect calculus but also anomalies in tooth structure to name a few, this skill is something I keep perfecting and working till this day,
Spring 2022 Semester
- These are evaluations from the beginning of the Spring 2022 semester where I had been practicing a great deal at home and always keeping in mind what guidance I received, as it is evident my scores were now improving to 4s
- This is an evaluation on exploring on a typodont in which I kept in mind to stay in line and not cross the line of occlusion.
- As I honed my skills I would always strive to help others and practice after clinic in order to build muscle memory.
- This was one of the same evaluations from the previous semester for probing on typodont. In the previous process evaluation I received a 3 out of 4, which is still a pass but in this level we are expected to have really perfected our skills. In this evaluation I received a 4 and some positive commentary on my performance.
- I really wanted to perfect my skill before probing on patients in order to record accurate pocket depth charting for periodontal assessments.
- This was an evaluation for actual periodontal probing on a patient in which I was able to use my skills I learned on my typodont in real life situation. I did receive a 4 out of 4 for this evaluation following the previous guidance from my professor(s).
- This is my midterm report from my advisor letting me know how professionalism was being graded by my instructors. I received excellent ratings from my instructors in all categories and positive regard. I am grateful for these words of inspiration and continue to maintain the same integrity coming into senior year.
